Additional to the claim form below, the insurance is offering both employees and their dependants to submit their claims online from the following .
It is also suggested to retain the original documentation in case further clarifications are required. It is also encouraged to include the IBAN details on the form so that settlement of such claims are made directly into the bank account.
Those employees / dependents that opt for cheque payments will be charged €4 that will be deducted from the claims settlement.
